Student DSG Application (Must Be Signed By Student & Parent)
To be eligible for the Dedicated Service Graduate Program, the application must be turned in prior to beginning your service hours. Please click on the link below to access the Dedicated Service Graduate Program Application. You will need to print the application and turn it in to your DSG campus coordinator.
Please click on the link below to access the Student Service Verification Form. You will need a copy of this form to take to each community service event. Copies of all completed verification forms, with a Service Summary Cover Sheet, need to be turned in to your DSG campus contact on either November 5, 2010 or March 4, 2011 to receive your DSG t-shirt and definitely on April 1, 2011 to conclude the DSG hours collection for the 2010-2011 school year.
